New Patient Update & Launch of Under 16s Rapid Health Form

New Under 16s Rapid Health Form – Launching Wednesday, 12th February

From Wednesday, 12th February, parents and guardians will be able to submit requests for children under 16 via the new Under 16s Rapid Health Form, making access to care more convenient.

  • The form asks five free-text questions to provide key details about the child’s health concern.
  • Every request is manually triaged by a GP, ensuring appropriate prioritisation.
  • An appointment is not booked at the time of submission – the practice will review the request and contact the patient, parent, or guardian on the same day.

CQC Inspection Announcement

We want to let you know that Modality Mid Sussex is being assessed by the Care Quality Commission (CQC) as part of their ongoing commitment to ensuring high-quality patient care.

πŸ” Assessment Start Date: 24th February 2025

The CQC is the independent regulator of health and social care services in England, and they will be evaluating various aspects of our care, including:

βœ… Access to appointments and services
βœ… Quality of clinical care and treatment
βœ… How well we listen and involve patients
βœ… Staff professionalism and service standards

As part of the inspection process, the CQC welcomes feedback from patients who have used our services in the past 14 months. Whether you have had a positive experience or have constructive feedback, your views are invaluable in shaping the future of our care.

Extended Hours

We offer appointments outside of core opening hours on Saturday mornings, alternating across all practice sites. Please contact reception for further details.

Out of Hours and Emergencies

For health information and advice, or if you need medical help fast when we are closed, call NHS 111. NHS 111 is the number to call when you need medical help fast but it’s not a life threatening emergency.
