New Under 16s Rapid Health Form – Launching Wednesday, 12th February
From Wednesday, 12th February, parents and guardians will be able to submit requests for children under 16 via the new Under 16s Rapid Health Form, making access to care more convenient.
- The form asks five free-text questions to provide key details about the child’s health concern.
- Every request is manually triaged by a GP, ensuring appropriate prioritisation.
- An appointment is not booked at the time of submission – the practice will review the request and contact the patient, parent, or guardian on the same day.

Extended Hours
We offer appointments outside of core opening hours on Saturday mornings, alternating across all practice sites. Please contact reception for further details.
Out of Hours and Emergencies
For health information and advice, or if you need medical help fast when we are closed, call NHS 111. NHS 111 is the number to call when you need medical help fast but it’s not a life threatening emergency.
In life threatening emergencies such as chest pain and severe shortness of breath you should call 999
